  • T4 slips for Employment income (i.e. pay cheque stubs)
  • T4A, T4OAS and T4AP for Old Age Security and CPP benefits
  • T4RSP, T4RIF and Other pensions and annuities
  • T4E for Employment Insurance benefits
  • T5007 for Social assistance payments
  • T5007 for Workers’ compensation benefits
  • T3 and T5 for Interest and dividends
  • Receipts for Deduction from Income and Tax Credits
  • Brokers note for Sale or deemed sale of stocks, bonds etc.
  • Support for a child, spouse or common-law partner
  • T4 RRSP for contributions made during the period applicable to year 2009
  • Professional fees or Union dues
  • Tool expenses (trades persons)
  • Medical expenses
  • Tuition/Education amounts for qualifying students
